Solution:
  1. Confirm you have authenticated via DUO
    1. if you did receive the DUO request, then you may not have actioned it in time; try again
    2. if you did not receive the DUO request, try one or more of the below steps and then try again:
      1. closing the DUO app and re-opening it on the device
      2. changing your device from wifi to cell data
      3. turn your device off and back on again
      4. contact FOS Support so that your DUO setup can be re-activated
